A new species, Otiorhynchus (Lixorrhynchus) milazzoi n. sp., from western Sicily is described. The specimens were collected died and are missing of the genitals. It is the first finding of this subgenus in Sicily and greatly extends its distribution southward. The genus rank of Ioniorhynchus Magrini, Meoli&Abbazzi, 2005, wrongly considered a subgenus of Otiorhynchus Germar, 1822 (Osella, 2008a) is re-instated on the basis of the clearly different male genitals of the two genera.
